APHL (Animal and Plant Health Laboratories) Is part of AVA (Agri-Food Veterinary Authority of Singapore).
AVA is a agricultural government agency in Singapore (you have a similar agency in Taiwan too. And, APHL is one of the many depts in this agency.
AVA就像是台灣的農委會，APHL是農委會裡面的實驗室。

Dear Stella,
Regarding your enquiry, the following is the procedures for submitting samples to APHL.

寄血清到新加坡實驗室的程序如下

1        The vet will first have to apply for AVA-accreditation by sending us a copy of his vet qualification & licence to practise (in Taiwan) via fax (65 63161090) or email. This will take about 2 days to process. This is an one-off application. (Note: We have received blood from Taiwan before, do check with us or the vet if he/she is accredited.)

首先，台灣的獸醫師必須經由填寫獸醫師執照(影本)以及合格獸醫院資料如下圖(總共三頁)，然後傳真+65-63161090或email，這個手續時間大約需要兩天。之後會得到新加坡農委會的認證(AVA accreditation)。(Just email Amy a copy of the vet qualification and license and Format_Vet will do.)

 

2        The accredited vet will then apply for an AVA import permit online (instructions attached). Please also fax/email us the  completed submission form and the vet health certificate (format attached) as supporting docs for the permit approval processing. This will take about  2-3 days.
接著，經過新加坡農委會認證的獸醫師本人必須上網申請輸入血清許可證，申請步驟有一份pdf檔可以參考，完成以後，需要把Submission Form for Companion和Vet health certificate(如下圖一、二)傳真或email過去。這個步驟也需要兩到三天。

Fill in the vet as the applicant. The vet health certificate is Format_of_Vet_Cert_for_Samples_from_Dogs&Cats. The submission form is CompAniform040607.

3        Once permit is approved, payment for permit (SGD 22) can then be made on-line. The permit can be printed out to accompany the parcel. Pls also fax us a copy for reference.

許可通過以後，上網支付新加坡幣22塊，把許可證印出來以後，隨著寄血清的盒子寄出。同時也copy一份傳真/email至AVA。
實際辦理：費用 44塊新加坡幣，上網付費前需要有許可證編號和當初申請獸醫師的護照號碼
4        Pls inform APHL of the consignment details once parcel has been sent out.

血清包裹寄出以後，通知APHL(也就是Amy)

5        APHL will fax/email you the invoice for the lab test fee for on-line payment (via credit card) or bank draft.

APHL街到通知以後，會透過傳真或email寄送血清測試實驗費用，必須上網或銀行繳納。

6        Lab results will be faxed & mailed to the submitting vet within a week after the receipt of samples.

血清檢查結過會透過傳真或mail方式寄到獸醫院。

7        And yes, we can do all 4 tests. Cost of the lab tests is as follows.

四種血液測驗費用如下，以新加坡幣計算。
        • ehrlichiosis - S$71.40 艾利西體
       • brucellosis - S$53.55 布氏桿菌
       • leishmaniosis - S$74.90  萊什曼原蟲
       • leptospirosis - S$53.55 鉤端螺旋體
